COVID and Flu Immunisation Letter and Consent for Students aged 12-15
Dear Parents/Carers
Please find below a link to a letter from Sirona Healthcare regarding the COVID and Flu Immunisations available in the Academy on 23rd and 24th November 2021.

There are separate consent forms for the flu vaccine and COVID-19 vaccine. Please complete both forms, indicating whether or not you wish your child to have each of the vaccines. If your child has already received their COVID-19 or Flu vaccines you do not need to return these forms.
Links to the two consent forms are below. If you don’t have access to the internet, you can request a paper copy from your child’s school.

Please complete both forms by midnight on Saturday 20th November 2021. If we do not receive your completed consent forms we may contact you in order to discuss your wishes for your child.
