Dear Parents/Carers

Please find below a link to a letter from Sirona Healthcare regarding the second dose of  COVID vaccine available in the Academy on Monday 7th March 2022.

Please complete the form, indicating whether or not you wish your child to have the vaccine.

Please complete the form by midnight on Wednesday 2nd March in order to receive the vaccination at the school session.
